What to Expect

  • Please bring all necessary paperwork/documentation. This may include identifying information such as a drivers license, as well as pertinent medical records- clearance letters, exemption documentation, labs and/or imaging results
  • Bring any prescription glasses/contacts, hearing aids, limb prosthetics, and any prescription/over-the-counter medications
  • You will be asked to complete a Medical Examination Report Form (this form outlines any past or current health conditions)
  • Expect to have your blood pressure, hearing, vision, and urine screened (Please note, the Urine Test is not a drug test. It is a test to screen for any protein, glucose, and/or blood in the urine which may be  an indicator of an underlying condition such as diabetes and/or kidney disease).  A Fingerstick blood sugar test may be obtained if glucose  is noted in your urine
  • Expect to have your lungs, eyes, ears, heart, mobility, and neurologic status assessed
  • Be prepared to explain any “yes” or “unsure” answers marked on your questionnaire
  • Please wear loose fitting clothing
  • Any falsifying or misleading information may result in disqualification
  • For the best outcome, please get a good nights rest, and refrain from any excessive alcohol, nicotine, salt, and sugar at least 2-3 days prior to exam
  • Payment is for the exam, not for the “exam decision” of whether you pass or fail. All payments are due at the time of service
